PARTICIPANT WAIVER, RELEASE OF LIABILITY, AND ASSUMPTION OF RISK AGREEMENT

ACKNOWLEDGMENT OF RISKS

I understand that participation in puppy yoga activities involves inherent risks, including but not limited to:

• Physical exercise and yoga-related injuries.
• Slips, trips, falls, or other accidents.
• Interaction with puppies, including scratches, bites, nips, allergies, or other animal-related injuries.
• Exposure to animal hair, dander, saliva, or other allergens.
• Unforeseen behavior by animals despite supervision and safety measures.

I voluntarily choose to participate despite these risks.

HEALTH DECLARATION

I certify that:

• I am physically capable of participating in yoga and related activities.
• I have consulted a medical professional regarding any health concerns that may affect my participation.
• I will immediately inform staff if I experience discomfort, pain, dizziness, or any medical issue during the session.
• I understand that Puppy Yoga Lounge is not responsible for determining my fitness to participate.

ASSUMPTION OF RISK

I knowingly and voluntarily assume all risks associated with participation in puppy yoga sessions, including risks arising from interaction with animals, physical activity, equipment, premises, and other participants.

RELEASE OF LIABILITY

To the fullest extent permitted by law, I release, waive, and discharge Puppy Yoga Lounge, its owners, directors, employees, contractors, instructors, volunteers, affiliates, and animal handlers from any and all claims, demands, causes of action, damages, losses, liabilities, costs, or expenses arising out of or related to:

• Personal injury;
• Illness or allergic reactions;
• Property damage or loss;
• Animal-related incidents;
• Participation in any activity at Puppy Yoga Lounge.

This release applies whether caused by negligence or otherwise, except where prohibited by law.

PROPERTY DAMAGE

I accept responsibility for any damage I may cause to the facility, equipment, furnishings, puppies, or property of Puppy Yoga Lounge through intentional, reckless, or negligent conduct.

PHOTOGRAPHY AND MEDIA CONSENT

• I consent to photographs and videos being taken during sessions and authorize Puppy Yoga Lounge to use such images for marketing, advertising, social media, website content, and promotional purposes without compensation.

PUPPY WELFARE POLICY

I agree to:

• Handle puppies gently and respectfully.
• Follow all staff instructions regarding animal interactions.
• Refrain from actions that may stress, frighten, or harm any puppy.
• Understand that staff may remove me from the session without refund if I fail to comply with animal welfare guidelines.

REFUND AND CANCELLATION POLICY

I acknowledge that all bookings are subject to Puppy Yoga Lounge’s cancellation and refund policies as published on its website and booking platform.

MINORS

Participants under the age of 18 must have this waiver signed by a parent or legal guardian.

GOVERNING LAW

This Agreement shall be governed by and interpreted in accordance with the laws of the Province of British Columbia and the laws of Canada applicable therein.

ACKNOWLEDGMENT

I have carefully read this Waiver and Release Agreement, fully understand its contents, and sign it voluntarily. I understand that by checking the electronic box and checking out I am giving up certain legal rights.

ELECTRONIC SIGNATURE AND ONLINE BOOKING CONSENT

I understand and agree that by checking the acceptance box, clicking “I Agree,” completing an online booking, making payment, or otherwise electronically accepting this Agreement, I am providing my electronic signature and acknowledging that it has the same legal effect as a handwritten signature.

I further acknowledge and agree that:

• I have had sufficient opportunity to read and understand this Waiver, Release of Liability, and Assumption of Risk Agreement before accepting it.
• My electronic acceptance constitutes my agreement to be legally bound by all terms and conditions contained herein.
• I understand that my participation in any Puppy Yoga Lounge activity is conditional upon my acceptance of this Agreement.
• I consent to the storage and retention of my electronic signature and booking information by Puppy Yoga Lounge for record-keeping and legal purposes.
• I agree that no certification authority or third-party verification is necessary to validate my electronic signature, and the lack of such certification shall not affect the enforceability of this Agreement.

By proceeding with an online booking, checking the acceptance box, and/or completing payment, I acknowledge that I have read, understood, and agreed to all terms of this Agreement and voluntarily accept its provisions.
